1: Get Started with Online Application Assistance

Some drivers prefer to handle the renewal process by mail, rather than taking advantage of assistance services, visiting the MVD in person, or via available online services. Service by mail is available with limitations. Read below for additional details or visit your local MVD.

3: Via a MVD office

If you prefer to access MVD services in person or your specific needs require an in-person visit, most, if not all, driver license services can be handled at a physical MVD office location. Varying MVD offices sometimes supply different types of services so you will need to check before your visit to make sure the MVD closest to you provides the service you require. Whether your drivers license was lost or stolen, the first thing you should do is generate and download your replacement form and checklist. These documents will help save time when you visit the MVD. Replacing your Arizona drivers license involves completing the following:

  • Filling out the necessary replacement form
  • Paying a fee of $12

Below are a few steps to replace an Arizona drivers license. The Arizona MVD is responsible for issuing a new drivers license, but the process needs to be followed before a replacement license can be issued.

Replacing Your Arizona Driver License Online
Applicants can visit the Arizona MVD site and follow the prompts to guide them through the process of getting a replacement or duplicate drivers license. Once you have completed this process and submitted the required fee by credit card, you will receive your duplicate card via mail.

By Phone
To replace a Arizona driver’s license over the phone, call (877) 301-8093, Monday through Friday, between the hours of 8 a.m. and 5 p.m. Officers are also open on Saturdays between the hours of 8 a.m. and 2 p.m. 

In Person
If you are visiting the MVD in person, you will need to submit:

  • At least two types of valid government issued identification - one of these will need to show your birth date. 
  • At least one of the forms of identification should have a photograph.
  • If the applicant does not possess photo identification, the Arizona MVD will require a third form of identification. 

Ideal identification shows the following information:

  • Name
  • Address
  • Date of birth
  • Age

Arizona Drivers License Replacement Out-Of-State

If your drivers license was lost, stolen, or damaged while you were out of the state, or country, you will need to follow the instructions listed above to replace it. Under normal circumstances, the replacement license is mailed to the address on file on the same day, unless the request was made after 4 p.m., where it will be mailed the next day.
